This notice and form is for you!
As previously notified, from 3 June 2024 FENZ will pay the NZPFU Protect Income Protection Insurance premiums direct for NZPFU members whose substantive positions are covered by the collective employment agreement. The 3 May NZPFU notice which explains the changes fully can be accessed here.

This form is for you if:

- You now want to sign up to the NZPFU Protect Income Protection Insurance from 3 June 2024 when FENZ is paying the insurance premiums direct to the employer. Please note you will not have cover before 3 June 2024 as the premiums would not have been paid before then.
Please complete the attached form and email it to wellington@nzpfu.org.nz. The NZPFU will make sure FENZ and Protect get a copy of your form so you are included in the FENZ bulk billing from 3 June 2024. If we do not receive your completed form before 3 June 2024 you can still join anytime and you will be included in the bulk billing from the time your form is received.
Once your forms are received you will receive confirmation of your membership to the insurance and a welcome pack from Protect.
If you are already a member of the NZPFU Protect Income Protection Insurance you have to sign a different form to be included in the bulk billing arrangement which has been sent to you by email and can be accessed via last Thursday’s national notice. You also have to make sure your weekly premiums are not in arrears.
